How about medical strain guagues.  They are little rubber tubes filled
with conductive liquids (Mercury I think, but you'd want a non-toxic one). 
As they stretch, the resistance increases in proportion to their length. 
You'd have to do some signal processing to turn them into clicks, but that's
probably not insurmountable.

I used to work in a sleep research clinic.  They use strain gauges to test
for physiological impotence...  use your imagination.

Boy, wearable design is really interesting.  Is anyone forming a group or
incubator for this stuff? Let me know.
