I still don't understand what you mean by an MPEG2 stream.
The way I understand this is that a transport stream carries multiple program
streams, which in turn are made up of multiple packetised elementary streams
(PES). Generally there are at least two, audio and video.
transport and program streams carry program specific ( PSI) information to tell
you how to extract the individual program streams and PES.

I may be misunderstanding you because you are talking about files, and my
experience of MPEG2 is in a broadcast medium.
