It appears that the AD624AD can be ordered online Analog Devices http://products.analog.com/products/links/models.asp?product=AD624 I also see them available in the UK from Dial Electronics http://www.dialelectronics.co.uk/a3.htm I have seen the disposable cardio electrodes avaialable at countless vendors on the web for cheap, and it appears that they can also be home brewed with a little creaitivity. The AD624AD produces analog output, therefore if you want to digitize the signal an A/D converter is required.
