A Twiddler2 isn't very speedy for text input.  Have you tried using
a laptop with a standard keys and key spacing (such as a ThinkPad)
for taking notes, and an MS Natural keyboard when at home/work?  I
can only speak for myself, but this combo stopped the pain I had in
my hands.

Also, in quiet environments, on Windows, Voice Recognition is at
the "doesn't suck" stage.

I think for a wearable input device, and given the lack of better
alternatives, $200 is a fair price.  Just not a good fit for your
needs.
