Pager size is really really tiny... If I were you I'd look for embedded linux systems, or lower your expectations, as to how small you can build a wearable system. For something about the size of a pager, I'd try look at those DIY linux/open source phone site/groups. They seem to know their way around hardware around that size, and give it some pretty good capabilities. Other than that, unless you're talking about a pager size I don't know of, I don't think you have many options.
