Hey-
I bought one of these things about 6 mo's to a year ago, even if I did
lose the tiny thing :) .
I still have the documentation, however...
---

V & H driver supply voltages: -1v to +17v
video signal input pin voltages: -1v to +15v
Supply voltage: 13.5±0.5v
input pin voltage 2.8v

Pins
---
 1)Common voltage of panel
 2)G video
 3)R video
 4)B video
5)not connected (no pin--kind of makes the connector "keyed")
6)Clock pulse for H shift register drive
 7) " " " " " "
8)Start pulse for H shift register drive
 9)Drive direction for H shift register (H-normal, L-reverse)
10)improvement pulse for uniformity
11)Enable pulse for gate selection
12)Clock pulse for V shift register drive
13) " " " " " "
14)Start pulse for V shift register drive
15)GND for the H & V drivers
16)H & V driver power supply

The problem, however, is getting the darn thing connected. The "pins"
are exposed traces on the flexible, plastic-ish tft tail, which is about
1-1.5 cm wide, so the pins are really small.

Hope this is useful,
